At Angel Care VNA we approach all patients as individuals, carefully assessing the client’s physical, financial, social and psychological needs. We will work closely with caregivers and physicians to develop a personalized plan of care and assign a professional that best matches the needs of the client. The plan of care will outline the services, frequency and duration of visits, and will coordinate with your health care providers as necessary. After a caregiver is assigned, our agency performs routine follow-up visits at your home to make sure all expectations are being met.

Alzheimer's/Dementia Care

Our caregivers have the experience and training needed to provide appropriate, safe and individualized care in keeping with the distinct challenges often presented by patients living with memory impairments and associated behavioral issues. Our Alzheimer and Dementia home care program has been designed to maintain overall independence and quality of life in a loving and familiar environment.


Companion Care

Companion Care provides emotional support and companionship for seniors who are generally healthy and want to remain independent in their home. It can also include a range of non-medical services, including basic cooking, exercises, memory games, shopping, a ride to recreation centers, to a hair salon or to visit a friend or a loved one. It also includes cleaning and support that help make a senior’s life more manageable.


Diabetes Care and Education

Our Diabetes Care and Education program includes blood sugar monitoring, meal planning assistance, behavior management and coping skills, foot and eye care, and diabetes medication monitoring.


Medication Management

Medications prescribed by your physician, when taken correctly, support the healing process. Knowledge and education about medications are key to maximizing their safety and effectiveness. Our home care team can oversee the administration of prescribed medications, educate the patient and family on proper dosages, frequency and side effects, evaluate therapeutic responses to medications, assess for duplicate medications or drug interactions, and assist in developing a medication schedule to enhance compliance.


Skilled Nursing Care

Angel Care VNA provides highly skilled, compassionate nurses on a full, part-time or per visit basis, who work closely with the client, their physician, family members and care providers to maintain the client’s health, and prevent further illness and injury. Our nurses conduct patient and environment assessments, and coordinate care with all team members. They provide treatment and educate patients and other caregivers on the disease or injury process, treatment plan, medications, diet and nutrition, foot care, catheter and wound care.


Wound Care

Our skilled clinicians are experienced in caring for and managing a variety of wound conditions in order to speed recovery, increase patient independence and comfort, and reduce the incidence of wound recurrence, complications and infection. We provide care for common wound conditions, including pressure ulcers, vascular and diabetic ulcers, surgical wounds, fistulas, and skin breakdown due to incontinence.
